Signature Award Application

Being a "Signature" PTA means your local unit has advocacy and parental involvement as its highest priority. You are the PTA that has garnered the respect from peers that all others want to emulate. Harford County Council of PTA, Inc. wants to recognize and reward your PTA for putting forth outstanding effort in ensuring that the mission and objects of the PTA are the backbone of your unit. Complete this application form and submit by January 15 to:

SECTION A: Complete all items below. Circle completed item numbers.

  1. Unit provided name, address, and phone number for officers and delegate to HCCPTA Corresponding Secretary by June 30. Corresponding Secretary will verify.
  2. Unit has paid Council, State and National Dues by October 31. Council Treasurer will verify.
  3. Insurance and bonding premium was paid to council by August 31. Council Treasurer will verify.
  4. Bylaws have been reviewed and approved in the past three years. First Vice President will verify.
  5. Budget was created in accordance with PTA Cash Encounters and was approved by thegeneral membership. Date of approval: ________________. Attach copy of approved budget.
  6. PTA Objects are displayed or read at all meetings.
  7. At least one voting member (president, principal or delegate) attends HCCPTA council meetings and submitted reports to the local unit. (Must sign attendance record.)
  8. At least one delegate attended an HCCPTA Council workshop. (Leadership Training or Back to School). Council Secretary will verify.

  13. An audit of records was performed. Attach copy of the last audit report.
  14. Minutes were recorded at each Executive Board and General Membership meeting. Attach a copy of one set of minutes.
